You probably want to select My Use Only, unless your computer has other users that you want to grant access to this connection.
Click Finish and it'll bring up the prompt for your user name and password. You may need to add the domain 'iam\' before your username to connect properly. You have the option of saving your username and password. To connect in the future, going to control panel>network connections, will show the connection name you picked in step 6. Double clicking the name will connect you to the VPN.
If you only follow the last 9 steps, you won't be able to access the internet once you've connected to the VPN. To fix that, click the properties button on the username / password prompt.
Then click the networking tab, make sure Internet Protocal (TCP/IP) is selected, and click the properties button
Click the Advanced button
unselect the checkbox for Use default gateway on remote network
Then click OK a whole bunch of times and you'll be good to go.
