Request a Certificate of Insurance for CTVA productions off campus. Additionally, the Location Agreement must be completed. If you’re also applying for a permit, you must start the application process with the Chicago Film Office, Chicago Park District and/or a county Forest Preserve. The permit application must include Columbia College Chicago after your name, your loop email and the address of the department office: 1104 S. Wabash, 313, Chicago, IL 60605.
Please schedule a meeting with Sandy Cuprisin to review the process at least two weeks prior to production.
